| How does the client pay for these services? |

Depending on the referral source the client may receive services free of charge. In the case of services obtained from a Community Care Access Center (CCAC) services are 100% funded by the Ontario Ministry of Health and Long-Term Care. In some cases clients may also be funded for service through the Workplace Safety and Insurance Board (WSIB), Veterans Affairs Canada, Insurance Company or from other sources. Clients may also choose to pay for some services themselves. |

| What are the clients’ rights and responsibilities in treatment? |

At Pace, clients’ rights must be communicated, respected and protected at all times.
Clients have the right to:
- Be given information about the services they will receive
- Give or refuse consent to service
- Receive safe and professional care
- Expect privacy about their personal and medical information
- Have their ethics, culture, family values, race, religion, gender, finances, specials needs and age respected
- Take part in making service decisions
- Be free from mental and physical abuse
- Voice concerns about and request changes to the services they receive
- Know who to contact if they have concerns
Clients have the responsibility to:
- Be available for service
- Respect the therapist
- Make sure that their home is safe
- Use the equipment and supplies safely and correctly
- Call the Pace Homecare office in a timely manner if they have to miss an appointment
- Provide all necessary information to the therapist to help plan their care
- Be involved in and follow the care plan

| What is in-home or on-site therapy? |

Our therapists provide health and support services in the client’s home. The intent of ‘in home’ or ‘on-site’ therapy is to allow the client to remain in their home or place of residence while receiving care.
Pace therapists also provide service in a variety of settings in the community in addition to client homes. Our associates visit residents in Long Term Care Facilities (Nursing Homes), Retirement Homes, Assisted Living Retirement Residences, and Group Homes. Pace also provides services in adult day programs, workplaces, hospitals and schools. |

| How does a person get referred for service? |

Pace Homecare receives referrals from several sources:
- Government Agencies, such as Community Care Access Centres (CCAC), Workplace Safety and Insurance Board (WSIB) and Veterans Affairs Canada. Typically a referral from one of these agencies meets the following criteria:
- The person will have a medical condition where adequate treatment can be provided at home.
- The persons’ needs cannot be met on an ‘out-patient’ basis through a hospital or other provider.
- Family Physicians or other Health Professionals.
- Insurance Companies.
- Individuals or Families.
- In this case Pace Homecare then conducts an assessment to determine if treatment is appropriate and necessary.
- A Lawyer may refer the person for an assessment.

| Who would need or want to use our services? |

At Pace we provide care to a wide range of clients. Services may be provided for individuals who:
- Are acutely ill or recovering from an illness, surgery or injury
- Have a chronic illness or functional disability
- Have the potential for rehabilitation or may require services to maintain a level of functioning
- Require an assessment for insurance or legal purposes
- Require an assessment for an Assistive Device
- Are experiencing difficulties functioning in their home
- Are in need of advocacy or referral to community resources
- Are experiencing, social, financial or emotional issues related to illness

| What about the relationship between the family, caregivers and Pace? |

The therapists of Pace Homecare are always open to the input and ideas of those who care for the clients we see. An important part of our work is to support those in a caregiver role in any way that we can. |
